I struggle to write about what is heavy on my heart. We have followed the COVD-19 pandemic closely and that has been heavy on my heart as I witnessed the tragedy of that. I have felt somewhat speechless about the racial tensions we are experiencing right now.

Being a registered nurse, I was struck by a statement that came out this week by the American Nurses Association President, Ernest Grant. He spoke of the nurse's Code of Ethics and how it obligates nurses to be allies and to advocate and speak up against racism, discrimination, and injustice. This is non-negotiable.

While there has been a lot of coverage regarding the almost universal agreement by insurance companies to cover the cost of being tested for COVID-19, not much coverage has been published of the financial (medical) aftermath. What happens if you are positive? What happens if you wind up in the hospital? What happens if you find yourself on a ventilator and hospitalized? What about those bills? What if there are extensive complications?

According to the Kaiser Family Foundation, recent data suggests that patients who go to intensive care for treatment of COVID-19 stay there for an average of twenty days.

Last week, a study released by America’s Health Insurance Plans shared that their research shows that the average cost to treat a hospitalized COVID-19 patient is $30,000. Are you ready to pay your share of that bill? Will you be responsible for that bill?

May is Mental Health Awareness month and a time to take stock of our mental health status and needs. Since March of 2020, life as we knew it has changed dramatically. Pandemic Stress

Words such as "quarantine" and "social distancing" have become part of our vocabulary and roll off our tongues as if this is our new normal. The mental health effects of social isolation, constant fear, and anxiety related to getting the virus, losing a job, or working from home and being on lock-down will be far-reaching into the upcoming months and years. Individuals with preexisting mental health conditions such as depression and anxiety are reporting exacerbations in these conditions.

Nurse's Week During the Covid-19 Pandemic Has a  New Meaning

This year Nurse’s Week is being celebrated May 6th through May 12th. The nursing profession is held in very high esteem by the U.S. public. For the past 15 years, nursing has been voted the most trusted profession by the Gallup’s poll survey because of nursing’s ethics and honesty. The public views nurses as “angels” of the healthcare system and holds them in very high regard. Nurse’s Week has always been a time to honor and shine a light on nurses for what they do, but this year takes on an entirely new level of praise.

In looking at magazines and newspaper articles for the past six weeks of the pandemic, pictures of nurses are everywhere comforting patients, holding their hands, administering treatments and care. Despite being overworked, tired, and exhausted ,the pictures show seasoned professionals in masks and gowns on the front lines rendering care to patients.

The expectations for nurses during this time went beyond the physical care of the patient. There were so many circumstances during the Covid-19 pandemic that made taking care of patients unique and difficult.

There is a concern that people are not getting the healthcare they need in a timely manner due to fears about getting the COVID-19 virus in the emergency room (ER). It seems people think they need to avoid the ER because it is overcrowded with COVID-19 cases or they fear contracting the virus.

This is an understandable fear but the statistics are very concerning to me as a patient advocate, whose mission it is to ensure people get the best healthcare at the right time. A recent survey of nine major hospitals earlier this month showed that the number of severe heart attacks being treated in U.S. hospitals had dropped by nearly 40% since this pandemic started. This has led to physician’s being concerned that patients are not going to the ER for an emergency and are waiting too long to get attention, which could lead to more permanent damage or worse.

Another alarming statistic is that EMT’s in New York City reported a quadrupling of house calls for cardiac symptoms between March 30 and April 5.

The Covid-19 pandemic continues to move through the world and the U.S. unlike any disease we have seen before. Early on in the pandemic, the experts identified the risk factors that would predispose a person to getting Covid-19. This included people age 65 and older, people with underlying illnesses such as hypertension, heart disease and diabetes, men more than women and obese individuals.

The Covid-19 pandemic has proven to be an unprecedented public health event globally. So many patient advocates and healthcare workers are dealing with so many unusual and "first time" clinical management issues. The usual "playbook" of clinical management and navigation of patient care has been changed and altered almost hourly as the pandemic moves through the U.S.

Issues such as critical care management, including when to place a patient on a ventilator and what medications should be used for treatment have been developed on an “as-needed basis”. As with any new disease, treatments are being developed as the needs arise. To say that healthcare workers must be resourceful is an understatement.

As nurse patient advocates, the challenges we faced at Healthlink Advocates (HLA) were more community-based issues, such as how to get our clients to their procedures and appointments safely and where and how to get our clients tested for Covid-19, if needed.

You may be overwhelmed with the COVID-19 pandemic. Like many, you may feel somewhat out of control and that life is so unpredictable at this time. As patient advocates, we have spoken to new and existing clients who are concerned about their wishes for end of life care. This is not a fun topic and most of us tend to avoid it.
